Wilson Martino Dental of Doddridge County offers a sliding fee schedule program for those who are uninsured or under-insured. The sliding fee scale is based on your family size and income. This fee scale allows you and your family to pay a reduced fee for covered dental services at Wilson Martino Dental of Doddridge County's West Union (Marie Street) location.
The Enrollment Process:
To qualify for the sliding fee schedule, you will need to complete a Sliding Fee Schedule Application and provide proof of your income. Be sure to complete all questions, include information for EACH person living in your household and provide the required documentation as listed on the application/policy.
